School Physicals

What do you need to know?
- They have to be dated after May, 1, 2020

- The Athletic Office will hold exams at METRO on Wednesday, May 13
4:00-5:30 PM
Please be there at 4 PM
Cost: $25

- They need to be turned in to our Athletic Trainer, Kimberly Claybaugh
PLEASE KEEP A COPY FOR YOUR RECORDS
You can turn them in one of two ways.
1- Turn the paper copy in to the Athletic Office

2- Email a PDF COPY to kclaybaugh@metroca.com

FYI: There is a great App for your phone called CAMSCANNER. It is free. You can take a picture of your physical and it will convert it to a PDF that you can email to Kimberly.
 (This is helpful to use with all kinds of information. You can also fax from this app for a cost per page.)
