PLEASE KEEP A COPY FOR YOUR RECORDS
The OSSAA Form is on the links needed in the right hand column that you can print off and take to your exam.
You can turn them in one of two ways:
1- Turn the paper copy in to the Athletic Office
2- Email a PDF COPY to ktaylor@metroca.com
FYI: There is a great App for your phone called CAMSCANNER.
It is free. You can take a picture of your physical and it will convert it to a
PDF that you can email to Kimberly.
(This is helpful to
use with all kinds of information. You can also fax from this app for a cost
per page.)
